Welkare Medical Office Has Permanently Closed
We would like to begin by thanking everyone who contributed to the remarkable history of Welkare Medical Office:
our wonderful patients,
our staff members over the years,
the specialists with whom we worked closely,
and the health-care partners who supported us throughout the more than 40 years we have had the privilege of serving our community!
We are especially grateful to our patients for the trust they placed in us and for allowing us to be part of their health-care journeys. Thank you.
Records Requests & Transfers
Following the closure of our medical practice, Welkare remained available for an additional two months to help patients transition to new providers and arrange for their medical records. That
transition period has now ended, and Welkare Medical Office is permanently closed.
For patients who signed a consent authorizing the transfer of their records, those records have been sent to their new doctor or clinic. Patients who consented to transfer their records to Wharncliffe Medical or WharnOx Medical can be assured that those clinics have received their complete medical record and are working diligently to enter the information into their electronic medical record systems.
All remaining patient medical records have been transferred to an independent medical-record storage company for secure long-term storage. As previously advised, Welkare no longer has custody of or access to patient records and is unable to process record requests.

The records storage company will assist you with the request process and advise you of the applicable cost. Fees will depend on the size of your medical chart, but we anticipate that the cost may range from approximately $200 to $500.
It has been an honour and a privilege to serve our community. We are deeply grateful for the trust you placed in us over the years, and we will always remember our patients with great fondness and appreciation. We will miss each and every one of you and wish you and your families nothing but happiness, continued good health, and all the very best for the future.
