Update Regarding Medical Record Transfers

Welkare Medical Office permanently closed on May 31, 2026, following the retirement of Dr. Muia and Dr. Robinson.

We understand that some patients still require assistance completing their consent paperwork and arranging the transfer of their medical records. To help patients during this transition, administrative staff will be available at 268 Clarence St. for the days below only.

Office hours for administrative assistance:

Monday, June 29th, Tuesday June 30th and Wednesday July 1st

10:00 a.m. to 4:00 p.m.

Please note that this is for medical record transfer matters only. There will be no doctors on site, and staff will not be able to assist with medical appointments, prescription renewals, test results, or medical advice.

Patients who have not yet completed the required paperwork may attend during the above dates and times to:

1. Complete the Medical Records Preparation & Release Authorization form;

2. Receive information about the new clinic’s name and location; and

3. Review the available chart transfer options and applicable fee.

   
   

If you have already completed your paperwork and made payment, you do not need to attend the office again.

If you have not yet selected a location for your ongoing care, please call us or come into the office during the dates and times listed above so administrative staff can assist you.

We are doing our best to answer phone calls for transfer-related questions only. You may call us at 519-672-9898. Due to the very high volume of calls, you may not get through right away. If that happens, please continue trying or feel free to drop into the office during the above hours.
