Dear Patients,
Welkare Medical Office will be permanently closing as of May 31, 2026, as Dr. Muia and Dr. Robinson will be retiring from the practice of medicine.
It has been an absolute pleasure and privilege to care for you and your families over the past 40 years. We are deeply grateful for the trust you have placed in us and will always value the relationships we have built.
To ensure continuity of care, we have arranged for a new group of family physicians to assume responsibility for our patients effective June 1, 2026.
With your written consent, your medical records will be transferred directly to the new clinic to ensure uninterrupted care. Effective June 1, 2026, the new physicians will become the custodians of all transferred medical records. For patients who do not enroll but do not request otherwise in writing, the new clinic will act as the designated custodian for secure storage of records in accordance with Ontario privacy laws.
If you do not wish your records to be transferred to the new clinic, you must notify us in writing.
We ask that all patients please come into the Welkare Medical Office before May 11th, 2026 to:
Complete a Medical Records Preparation & Release Authorization form
Receive information about the new clinic’s name and location
Obtain details regarding transfer process & fee
Our staff will be happy to assist you, and the process takes only a couple of minutes.
We will continue to provide your care until May 31, 2026. Our goal is to make this transition as seamless and smooth as possible for every patient.
