Final Notice - Records Transfer

Important Final Notice Regarding Patient Medical Records

Although Welkare Medical Office officially closed on May 31, 2026, the office has remained open for nearly two additional months to help patients transition to their new clinics and arrange for copies or transfers of their medical records.

Please be advised that Friday, July 31, 2026, is the final day that Welkare staff will be available to assist with medical-record requests directly.

After July 31, all remaining patient charts will be transferred to an independent medical-record storage company for secure long-term archiving. Once this transfer has occurred, Welkare will no longer have direct access to the records or control over the retrieval process.

Request Your Records Before They Are Archived

Dr. Robinson and Dr. Muia have temporarily reduced the cost of obtaining or transferring patient records to help patients during this transition.

Once the records have been transferred to storage and archived, the cost of retrieving and preparing a chart will increase. Depending on the size of the chart and the work required to retrieve it, the estimated cost may be approximately $200 to $500.

Patients who have found a new doctor’s office, or who would like a personal copy of their medical records before the archival and retrieval fees take effect, should contact Welkare no later than July 31, 2026.

After the records are transferred to the storage company, all requests, processing times, retrieval requirements, and applicable fees will be handled by the storage provider and will no longer be within Welkare’s control.
