Additional Hours - Admin ONLY

Dear Patients,

Welkare Medical Office is now permanently closed as of May 31, 2026, as Dr. Muia and Dr. Robinson have retired from the practice of medicine.

We have previously sent notices regarding the office closure and the steps required for the transfer of medical records. Due to overwhelming requests from patients to remain open, admin will be on site at 268 Clarence St. for this week only to assist with consents and chart transfers only. There will be no doctors available as they have retired, it will be administrative staff ONLY!

The office will be open Monday June 8th to Friday June 12th from 10am to 4pm for consents and chart transfers only. Please note that staff will be available, but there will be no doctors on site.

Patients who have not yet completed mandatory paperwork are asked to come into Welkare Medical Office during this time and

-1) Complete the Medical Records Preparation & Release Authorization form

-2) Receive information about the new clinic’s name and location

-3) Obtain details regarding the transfer process options and fee. 

If you have already completed your paperwork and made payment, you do not need to come into the office again.

If you have not yet selected a location for your ongoing care, please call or come into 268 Clarence St during these times so staff can assist you.
