MUST READ BEFORE BOOKING

Please make sure to read and follow all the below instructions before booking an appointment online. Appointments booked incorrectly will be cancelled. Appointments missed because of a mistaken booking will be billed a missed appointment fee.

  1.  In the description of the appointment you must enter "OFFICE" for in office appointments and "PHONE" for phone call appointments, and then the reason why you need the appointment.
    For example: "OFFICE - sprained ankle", or
    "PHONE - meds renewal" 

     

  2. OHIP covers ONLY 1 appointment per day.
    Multiple appointments will be cancelled.

     

  3. Do not put messages to reception in the appointment information as they are not checked. If you feel your appointment is more time sensitive than is available, do not book online, call the office as we sometimes have last minute cancellations.
     

  4. If your doctor's name does not appear on Medeo for booking options, we could be booking further out than is available online, or the booking software is having technical difficulties.
    Either way please call our office to book.

     

  5. Please carefully read and select the correct TYPE of appointment
    (ie: in office, phone, mental health, physical, drivers physical, baby visit, OB etc.). 
    Incorrectly booked appointments may need to rebook.
    Do not just select whatever appointment is available. It must be appropriate.

     

  6. Please describe the reason for the appointment in as much detail as possible to help the doctor.
     

  7. Due to factors out of our control
    Phone call appointments will be a MAXIMUM of 5 minutes
    and most
    In Office appointments will be a MAXIMUM of 10 minutes.

     

  8. Whenever possible you should try to book an in person appointment, unless you have COVID / COVID symptoms / or your doctor has specifically requested you book a phone call.
     

  9. If you have COVID symptoms (cough, fever, difficulty breathing, sore throat, runny nose, shortness of breath, flu symptoms etc.) please make sure to test for COVID the day before and the day of your appointment. As long as both tests are negative, you may attend your appointment as normal (still wearing a mask). If any of the tests are positive, DO NOT come into the office. Call our office for next steps.
    If you do not have the ability to take a COVID test at home you MUST call our office to book.
    Thank you for helping us keep our immunocompromised patients safe.

     

  10. Due to COVID19 you MUST wear a mask into the office if you have symptoms (cough, fever, difficulty breathing, sore throat, runny nose, shortness of breath, flu symptoms etc.)
    If you do not have one, one will be provided to you at reception.

     

  11. We require 24 hours notice for appointment cancellations or reschedules. Please reach out to our office to reschedule as soon as you are able. Failure to do so, and or missed appointments will be charged a fee of up to $65.00.
     

  12. Please be reminded that as per the office policy patients who have missed THREE appointments may be dismissed from the practice, since the time is needed in the care of other patients.
     

  13. If you feel your concern cannot wait you can call/text our after hours at 519-672-8618, or go to their website LDRS.ca, or go straight to the hospital.
